Privacy Code

The Privacy Act for individuals within provinces became effective January 1st, 2004. This Act sets out ground rules for how private sector organizations may collect, use or disclose personal information in the course of commercial activities.

The Erin Mills Soccer Club (EMSC) respects your right to personal privacy. When we collect your personal information you can be confident that we will handle and store it in a safe secure manner to protect information from unauthorized access or disclosure.

This notice describes our PRIVACY CODE practices.

Erin Mills Soccer Club requires personal information from its players, coaches, managers, team staff and volunteers, and this personal information is obtained on the appropriate EMSC/OSA Registration Forms. This information is provided to the Ontario Soccer Association, and may be provided to Peel Halton Soccer Association and South Region Soccer League or any other leagues for which EMSC teams are registered or associated organizations for purposes of individual registration, and to our insurer in connection with insurance coverage.

EMSC does not share, give, rent, or sell client information to any organization or individual except to comply with laws, or regulations of a governmental or regulatory body or in response to a valid subpoena, warrant, or order, or to protect the rights of EMSC or any of its players, coaches, managers, team staff, employees or volunteers.

Accountability

EMSC has appointed a Privacy Officer as required by the Privacy Legislation with Policies and Procedures in place for the management of personal information. These are available upon request from the Clubhouse.

Why do we collect Personal Information?

We request only the minimum information required to complete Insurance and Registration requirements required by Ontario Soccer Association, Peel Halton Soccer Association, South Region Soccer League, its insurer, EMSC’s professional advisors including but not limited to its lawyers and accountants.

EMSC requests Player name, proof of birth, health concerns, and soccer experience. Parents/Legal guardian information is requested if a player is under 18 years of age, or player information if over 18 years of age including; address, occupation, home phone, work phone, emergency phone number, and e-mail. Registration forms also assist the Club to gather information concerning possible volunteer work for the Club by members as coaches, directors, convenors, sponsors, or social committee members.

This information is used to organize and select teams for the indoor and outdoor soccer programs organized by EMSC, to determine skill level and experience of players, to draft players, to balance house league teams, to validate the age and residency of players, to organize technical skills development and training programs or clinics, to develop Player profiles for coaching purposes, to organize and deliver the programs and services offered to Members by the Club, and for other purposes within the Charter and mandate of the Erin Mills Soccer Club.

This information may be disclosed to and used by the employees, staff and directors of the EMSC as well as coaches, managers, conveners and other volunteers.

Health Concern requests ensure coaches are aware should any health problems arise during practice or games. Health information is provided only with the consent of the parent/guardian or the player, if over 18 years of age.

Police Volunteer Check

Required for all directors, conveners, coaches, managers, team staff and volunteers. The application once completed and returned from the police is brought to EMSC for verification. The Volunteer Check is retained by the individual.

Files are maintained on any information we have provided to you, employment applications, enquiries, complaints, compliments, insurance or other correspondence made by you and our response if any.

The Ontario Soccer Association requires that all player, coach, team staff, and volunteer registration files be maintained for seven years.

If a member requests disclosure in writing of the personal information collected by EMSC concerning that member or a Player for whom they are a parent or legal guardian EMSC shall disclose such information not later than 30 days after receipt of a written request.

EMSC reserves the right to charge an administrative of $25.00 for any such request. Furthermore, EMSC may deny such a request, if providing a response would result in the disclosure of personal information concerning a third party.
